There is no charge for establishing an account with Quest Diagnostics for your laboratory testing.
Please fill out a form for each physical location to ensure the designated representative for the location contacts you.
When the representative contacts you they will provide their email address and phone number. You will be able to contact them via those methods and they will be happy to help you.
You can still fill out the form, but please notify the representative if you believe you may have an account. They will be able to help determine if you have an active account. If you have an account, your rep will provide you with the account number.
